WHOの定義

緩和ケアとは

緩和ケアとは、生命を脅かす病に関連する問題に直面している患者とその家族のQOLを、 痛みやその他の身体的・心理社会的・スピリチュアルな問題を早期に見出し的確に評価を行い対応することで、 苦痛を予防し和らげることを通して向上させるアプローチである。


緩和ケアは・・・
  • 痛みやその他のつらい症状を和らげる
  • 生命を肯定し、死にゆくことを自然な過程と捉える
  • 死を早めようとしたり遅らせようとしたりするものではない
  • 心理的およびスピリチュアルなケアを含む
  • 患者が最期までできる限り能動的に生きられるように支援する体制を提供する
  • 患者の病の間も死別後も、家族が対処していけるように支援する体制を提供する
  • 患者と家族のニーズに応えるためにチームアプローチを活用し、必要に応じて死別後のカウンセリングも行う
  • QOLを高める。さらに、病の経過にも良い影響を及ぼす可能性がある
  • 病の早い時期から化学療法や放射線療法などの生存期間の延長を意図して行われる治療と組み合わせて適応でき、 つらい合併症をよりよく理解し対処するための精査も含む

Palliative care is an approach that improves the quality of life of patients and their families facing the problem associated with life-threatening illness, through the prevention and relief of suffering by means of early identification and impeccable assessment and treatment of pain and other problems, physical, psychosocial and spiritual.
Palliative care
・ provides relief from pain and other distressing symptoms;
・ affirms life and regards dying as a normal process;
・ intends neither to hasten or postpone death;
・ integrates the psychological and spiritual aspects of patient care;
・ offers a support system to help patients live as actively as possible until death;
・ offers a support system to help the family cope during the patients illness and in their own bereavement;
・ uses a team approach to address the needs of patients and their families, including bereavement counselling, if indicated;
・ will enhance quality of life, and may also positively influence the course of illness;
・ is applicable early in the course of illness, in conjunction with other therapies that are intended to prolong life, such as chemotherapy or radiation therapy, and includes those investigations needed to better understand and manage distressing clinical complications.

緩和ケアとは、治癒を目指した治療が有効でなくなった患者に対する積極的な全人的ケアである。痛みやその他の症状のコントロール、 精神的、社会的、そして霊的問題の解決が最も重要な課題となる。緩和ケア目標は、患者とその家族にとってできる限り可能な最高の QOLを実現することである。末期だけでなく、もっと早い病期の患者に対しても治療と同時に適用すべき点がある。

Palliative care is the active total care of patients whose disease is not responsive to curative treatment. Control of pain, of other symptoms, and of psychological, social and spiritual problems is paramount. The goal of palliative care is achievement of the best possible quality of life for patients and their families. Many aspects of palliative care are also applicable earlier in the course of the illness, in conjunction with anticancer treatment. (WHO 1990)

緩和ケアをめぐる言葉
ターミナルケア Terminal care

1950年代からアメリカやイギリスで提唱された考え方で、人が死に向かってゆく過程を理解して、医療のみでなく人間的な対応をすることを主張した。

ホスピスケア Hospice care

1960年代からイギリスで始まったホスピスでの実践を踏まえて提唱された考え方で、死に行く人への全人的アプローチの必要性を主張した。

緩和ケア Palliative care

1970年代からカナダで提唱された考え方で、ホスピスケアの考え方を受け継ぎ、国や社会の違いを超えて人の死に向かう過程に焦点をあて、 積極的なケアを提供することを主張し、WHOがその概念を定式化した。

支持療法 Supportive care

1980年代にアメリカやヨーロッパでがん治療から発展した考え方で、治療に伴う副作用の軽減や、リハビリテーションなど抗がん治療でない 様々な治療を指しており、緩和ケアと重なる概念である。

緩和医療学(緩和医学) Palliative Medicine

1980年代からイギリスで緩和ケアを支える学問領域として発展したもので、国際的にも緩和ケアを学問的に裏ずける医学や看護学等の専門領域のひとつとして確立している。

エンドオブライフ・ケア End-of-Life care

1990年代からアメリカやカナダで高齢者医療と緩和ケアを統合する考え方として提唱されている。北米では緩和ケアはがんやエイズを対象としたものという理解があり、がんのみならず認知症や脳血管障害など広く高齢者の疾患を対象としたケアを指している。
